When enrolling in the Emily’s Hope Substance Use Prevention Curriculum, your organization’s primary contact agreed to the following consent:
Survey Administration: I understand that our teachers are required to administer a pre-curriculum survey to students before accessing the Teacher Portal and are highly requested to administer a post-curriculum survey to measure progress. If you choose not to administer the surveys, the curriculum is available at a cost. Please contact Emily’s Hope for details.
Evidence-Based Status Acknowledgment: I acknowledge that while Emily’s Hope Prevention Curriculum is working towards obtaining evidence-based status, it is not currently recognized as such.
Invitation to Participate: You are invited to participate in a research study conducted by Emily’s Hope Substance Use Prevention Curriculum to improve substance use disorder understanding and prevention.
Consent for Research: By participating, you agree that data collected, including demographic and survey responses, will be used for program quality and curriculum effectiveness research. No identifiable individual information is collected, and all measures will be utilized to maintain confidentiality. There are no risks in participating beyond everyday life, and the information will be used solely for research by Emily’s Hope.

What Will Happen if You Take Part in the Study: Teachers are invited to administer a pre-curriculum and post-curriculum survey to students, directly related to the curriculum content.
What Are the Potential Benefits from This Study: Your participation will support the study to determine program quality and curriculum effectiveness for substance use disorder understanding and prevention.
How Will We Protect Your Information: All efforts will be made to keep demographic data and survey responses confidential.
How Will My Information Be Used After the Study: The information will be used solely for research purposes, and any published report will not include identifiable information.
How Will the Study Compensate You for Being Part of the Study: No compensation to participants is provided.
Your Participation in This Study is Voluntary: Participation is completely voluntary, and you have the right to withdraw at any time.

Shortly after completing the post-curriculum surveys, we can generate section-level reports so you can see student improvement in your classroom. If you’re interested, please get in touch with us after completing post-curriculum surveys.
What if my school doesn’t have computers or tablets to administer the surveys?
For research accuracy, we’re requesting schools administer the survey digitally. However, we understand each school is different and technology isn’t always readily available to students. Please contact our team to get printable surveys. You will need to scan and email or mail each of the paper surveys to us.
